Please note that it's important to refer to the official terms and conditions provided by the manufacturer (Lilly) for the most accurate and up-to-date information regarding the savings program for Mounjaro.
- Patients must have commercial drug insurance.
- Insurance coverage must not include Mounjaro.
- Prescription must align with FDA-approved labeling for Mounjaro. (Dx for Type 2 Diabetes)
- Eligible patients can receive savings of up to $575.
- Savings apply to a 1-month prescription fill of Mounjaro.
- A month is defined as 28 days or up to 4 pens.
- Maximum monthly savings allowed is up to $575.
- Separate maximum annual savings allowed is up to $3,450 per calendar year.
- Lilly reserves the right to terminate, rescind, revoke, or amend Card eligibility criteria and/or Card terms and conditions at its discretion.
- Changes to eligibility or terms may occur without notice and for any reason.
- Card expires, and savings end on 12/31/2023.
